Initial Visit
What to expect upon making an appointment

When you come to your initial visit, you will be greeted
and handed a new
patient form for you to fill out. Please fill out Pages 1 & 2
and sign where
needed. Please do not forget to fill out your full address,
along with referring
doctor information as it is integral since we use that information
to then contact
the referring doctor with post-evaluation notes. After you finish
filling the form out, you
will be asked to enter your room. Here you will be seated by one
of our staff.
The doctor will then come in and introduce himself and speak to
you regarding
why you have been referred or why you decided to visit. Probing
measurements
will then be taken. These measurements are taken to see if there
is any attachment loss or
loss of tooth supporting tissues. Although the instrument
looks scary, these measurements are not painful,
and are done quickly. With these measurements the Doctor can
see what needs
to be done and will assign you a treatment plan to follow. If
the Doctor feels
it is necessary, X-rays may also be taken.
NOTICE We don't want you to be scared,
but if you are, don't worry!
Our office has a clinical psychologist, Dr. Manrique-Reichard on staff.
She will provide you with stress management and relaxation techniques.
This will ease your mind and worries and may enhance recovery. Please
tell our secretary if you have any of these concerns
so that we may schedule you for relaxation prior to your treatment plan.
